COPD Exercises: Breathing Techniques and Physical Activity Guide

Why Exercise Matters in COPD

If you have COPD, shortness of breath might make exercise seem impossible. But here's the truth:

  • Exercise reduces breathlessness over time
  • It improves quality of life
  • It increases exercise tolerance
  • It reduces hospitalizations
  • It's one of the most effective COPD treatments
  • Breathlessness during exercise is expected—but it's safe and it gets better.

    Breathing Techniques

    Master these before increasing activity:

    Pursed Lip Breathing

The most important technique for COPD:

    1. Inhale slowly through your nose (2 counts)

    2. Purse your lips like you're blowing out a candle

    3. Exhale slowly through pursed lips (4 counts)

    4. Exhale should be twice as long as inhale

    When to use:

  • During exercise
  • When short of breath
  • During daily activities
  • As a regular practice
  • Diaphragmatic Breathing

Engage your diaphragm for more efficient breathing:

    1. Lie on back or sit comfortably

    2. Place one hand on chest, one on belly

    3. Inhale through nose—belly should rise, chest stays still

    4. Exhale through pursed lips—belly falls

    5. Practice 5-10 minutes, several times daily

    Coordinating Breath with Movement

    General rule: Exhale during exertion

  • Exhale when standing up
  • Exhale when lifting
  • Exhale when climbing stairs
  • Exhale when bending
  • This reduces the work of breathing during effort.

    Starting an Exercise Program

    Preparation

  • Use bronchodilator before exercise if prescribed
  • Warm up 5-10 minutes
  • Have rescue inhaler available
  • Check oxygen levels if you use supplemental O2
  • Setting Intensity

    Talk Test:

  • You should be able to talk (short phrases) during exercise
  • If you can't speak at all, slow down
  • If you can easily have a conversation, increase intensity
  • Rating of Perceived Exertion:

  • Target 3-4 out of 10
  • Some breathlessness is expected
  • Severe breathlessness (7+) means slow down
  • When to Stop

  • Severe shortness of breath that doesn't improve with rest
  • Chest pain
  • Dizziness
  • Unusual fatigue
  • Aerobic Exercise

    Cardiovascular training is essential:

    Walking

    Starting program:

  • Walk at comfortable pace
  • Use pursed lip breathing
  • Start with 5-10 minutes
  • Rest as needed (standing or sitting)
  • Progress by adding 1-2 minutes per week
  • Interval approach:

  • Walk 2-3 minutes
  • Rest 1-2 minutes
  • Repeat 3-5 times
  • Progress by extending walk intervals
  • Stationary Cycling

  • Good option for breathlessness
  • Start with no resistance
  • 5-10 minutes initially
  • Progress duration before intensity
  • Arm Ergometer

  • Upper body cardio
  • Especially important since arm activities often cause breathlessness
  • Start with 5 minutes, low resistance
  • Water Exercise

  • Excellent for COPD
  • Chest-deep water supports breathing muscles
  • Temperature should be comfortable (not hot)
  • Buoyancy allows easier movement
  • Strength Training

Strong muscles use oxygen more efficiently:

    Upper Body (Important for Breathing)

    Seated Rows

    1. Use resistance band

    2. Pull elbows back

    3. Exhale during pull

    4. 10-12 repetitions

    Shoulder Shrugs

    1. Hold light weights

    2. Raise shoulders toward ears

    3. Lower slowly

    4. 10-12 repetitions

    Wall Push-Ups

    1. Stand facing wall

    2. Exhale as you push away

    3. 10-12 repetitions

    Lower Body

    Sit-to-Stand

    1. Sit in chair

    2. Exhale as you stand

    3. Sit slowly

    4. 10-12 repetitions

    Heel Raises

    1. Hold support

    2. Rise onto toes

    3. Lower slowly

    4. 10-12 repetitions

    Seated Leg Extensions

    1. Sit in chair

    2. Extend one leg

    3. Lower slowly

    4. 10-12 each leg

    Guidelines

  • 2-3 sessions per week
  • Light to moderate weights
  • Higher reps (12-15)
  • Exhale during effort
  • Rest between exercises
  • Flexibility Exercises

    Maintain chest wall mobility:

    Chest Stretch

    1. Stand in doorway

    2. Arms on frame

    3. Lean forward

    4. Hold 30 seconds

    Trunk Rotation

    1. Sit or stand

    2. Rotate upper body each direction

    3. Hold 15-20 seconds each

    Side Bending

    1. Sit or stand

    2. Reach one arm overhead, lean to opposite side

    3. Hold 15-20 seconds each side

    Shoulder Rolls

    1. Roll shoulders forward, then backward

    2. 10 each direction

    Managing Breathlessness Positions

When you get short of breath:

    Standing:

  • Lean forward slightly
  • Rest hands on thighs or surface
  • Use pursed lip breathing
  • Sitting:

  • Lean forward
  • Rest elbows on knees
  • Or lean back with arms supported
  • High Side-Lying:

  • Lie on side
  • Prop up with pillows
  • Good for nighttime breathlessness
  • Daily Activity Tips

    Pacing:

  • Break activities into smaller tasks
  • Rest between activities
  • Don't rush
  • Energy Conservation:

  • Sit when possible
  • Use rolling cart for carrying items
  • Keep frequently used items accessible
  • Breathing During ADLs:

  • Exhale during bending, lifting, reaching
  • Use pursed lip breathing
  • Take rest breaks
  • Sample Weekly Program

    Daily:

  • Breathing exercises (10 minutes)
  • Stretching (5-10 minutes)
  • 3-5x/Week:

  • Aerobic exercise (15-30 minutes)
  • Progress as tolerated
  • 2-3x/Week:

  • Strength training (20 minutes)
  • Pulmonary Rehabilitation

If available, pulmonary rehab programs provide:

  • Supervised exercise
  • Education
  • Breathing techniques
  • Nutritional guidance
  • Psychological support
  • Research shows significant benefits. Ask your doctor for a referral.

    The Bottom Line

COPD doesn't mean you can't exercise—it means you must exercise:

    1. Master breathing techniques first

    2. Start low, progress slow

    3. Some breathlessness is expected and safe

    4. Strength training helps breathing muscles

    5. Maintain flexibility in chest wall

    6. Consider pulmonary rehabilitation

Exercise is one of the best treatments for COPD. Your breathing can get better.
